List the Order Number, Quantity, Size, Price and Total for each item ordered. Follow the Shipping and Handling Rate Chart to apply the appropriate shipping and handling charges to your order. Michigan residents add 6% Sales Tax.

Please send your item(s) back to us via insured and prepaid U.S. mail or UPS. Shipping and handling fees are not refundable. Returns will be refunded by check within four weeks of receipt.
