First and Last Name, Complete mailing address with Street Number and Name, State, Zip and Daytime Telephone Number. Please include your email address if you'd like to be notified when your order is sent.
List the Order Number, Quantity, Size, Price and Total for each item ordered. Follow the Shipping and Handling Rate Chart to apply the appropriate shipping and handling charges to your order. Michigan residents add 6% Sales Tax.

Please send your item(s) back to us via insured and prepaid U.S. mail or UPS. Shipping and handling fees are not refundable. Send exchanges or returns to: Circle®Brands, 186 S. Georgetown Sq. Bldg. 8, Royal Oak, MI 48067. Exchanges will be made within four weeks of receipt. Returns will be refunded by check within four weeks of receipt.
